Permit Applications & Forms

Listed below are applications, forms and checklists that you can download from this site. You can fill these out ahead of time and simplify your application process. The checklists, calculation tables and requirements provide further information on what you need to submit with your application. If you have any questions, please call (561)243-7200.

Effective October 1, 2010, per State Statute 468.631 and 553.721, the Dept of Business & Professional Regulation (DBPR) and Department of Community Affairs (DCA) fees will now be collected on all permits that are governed under the Florida Building Code. The total surcharge is 3% of the permit fee charged. Minimum fee is $4.00. The square foot calculation will no longer be used.
